Monoclonal vs polyclonal free light chain assays.

Am J Clin Pathol 2009;131:897-903 901 © American Society for Clinical Pathology relative to Freelite (Figure 1). Both patients had negative results for urine BJPs, further undermining the validity of the ELISA assay results. It is possible that the monoclonal antibodies are cross-reacting with the reported IgG λ and IgA λ monoclonal intact immunoglobulins in these patients, leading to falsely raised λ FLC results. Suggestions by Davern and colleagues1 that Freelite antisera regularly underestimate results due to missing a wide variety of λ subgroups, are affected by instrumentation and “unknown” technical issues, and are confounded by interference from intact immunoglobulins are not supported by their data. The results reported and views expressed in the article should be compared with the scientific consensus in the many, larger, and more rigorous evaluations of Freelite already published.

Diagnostic accuracy of monoclonal antibody based serum immunoglobulin free light chain immunoassays in myeloma cast nephropathy
UNLABELLED BACKGROUND The development of serum immunoassays for the measurement of immunoglobulin free light chains has led to a paradigm shift in the diagnosis, assessment and monitoring of patients with plasma cell dyscrasias. The impact of these immunoassays which employ polyclonal antibodies was most notable for those patients who were previously classified as non-secretory multiple myel...
متن کاملAre renal reference intervals required when screening for plasma cell disorders with serum free light chains and serum protein electrophoresis?
Renal impairment and polyclonal hypergammaglobulinemia may abnormally increase the serum free light chain (sFLC) ratio, giving false-positive results with current reference intervals. We measured sFLCs with concomitant serum protein electrophoresis (SPEP) and urine protein electrophoresis (UPEP) in 281 patients. Results were interpreted relative to renal function (serum creatinine concentration...
متن کاملClinical comparison of new monoclonal antibody-based nephelometric assays for free light chain kappa and lambda to polyclonal antibody-based assays and immunofixation electrophoresis.
BACKGROUND New monoclonal antibody-based assays for serum-free light chains (FLC) have become available. METHODS In a clinical study with 541 patients, the new N Latex FLC assays were compared with the Freelite FLC assays and immunofixation electrophoresis (IF). RESULTS Comparison of the different FLC kappa (κ) assays showed a slope of 0.99 with a deviation of 5.0%, rs=0.92, for FLC lambda ...
متن کاملDevelopment and characterization of polyclonal antibody against human kappa light chain in rabbit
Polyclonal antibodies against kappa light chain are used to diagnose diseases producing free light chain. The kappa and lambda light chains are products of immunoglobulin synthesis and released into the circulation in minor amounts such as serum, cerebrospinal fluid, urine and synovial fluid in normal condition. The purpose of this study was the production and purification of polyclonal immunog...

Comparison of free κ and λ light chain immunoassays and total (bound and free) κ and λ light chain immunoassays To the editor: We read, with interest, the article by Dr. Zamora-Ortiz, et al. Poor performance of the total kappa/ lambda light chain quantification in the diagnosis and follow-up of patients with multiple myeloma on the comparison of the total serum light chain assay (sLC), SPE and ...
